Practitioner Rebate Program Terms & Conditions
Welcome to the Practitioner Rebate Program (“Program”). Please read these Terms and Conditions (“Terms”) carefully. The Patient Fulfillment is an added service provided by Spectrum Innovations (“SII”) on behalf of its partner Healthcare Practitioner (“HCP”) to their Patients.
1. ELIGIBILITY & ENROLLMENT
- This Program is open to all registered HCP of SII.
- To enroll in this program, HCP must complete the form and submit a copy of BIR Form 2303 or Certificate of Registration (COR).
2. REBATE CREDITS
- HCP enrolled in this Program are entitled to earn up to 20% rebate credits with each patient orders.
- 20% rebate will be credited to the HCP for VATable sales; while 5% rebate will be credited to the HCP for NON-VATable sales, applicable to Senior Citizen & PWD patients with valid ID.
- SII may offer discounts and promotions to patients at its discretion. All rebates will be calculated based on the invoiced amount after any discounts or promotions have been applied.
- A monthly report shall be sent to the HCP’s registered email, which will include a summary of patient orders and cumulative rebate total.
- SII reserves the right to correct at any time the computation of rebate which it has deemed to be erroneous.
3. PAYOUT
- HCP at any time may initiate the rebate payout process by sending an email indicating the desired rebate amount and the preferred payout date. HCP shall allow a minimum of five (5) business days for the processing of rebate payout.
- All rebates are subject to withholding tax.
- HCP agrees to provide an official receipt and a signed copy of BIR Form 2307 upon the release of rebate. HCP may choose to arrange their own courier service or schedule a messenger to faciliate the document exchange.
- SII reserves the right hold the release of rebate until the official receipt and signed copy of BIR Form 2307 are available or can be provided.
- Rebates may be accumulated within one (1) calendar year. All unrelesed rebates will automatically be disbursed by January the following year.
4. PATIENT FULFILLMENT
- All patients shall register using the HCP’s unique practitioner code proir to ordering.
- Patients at any time may place their orders through the Patient Fulfillment site, or directly with SII.
- All patients will be charged Standard Retail Price (SRP) for the products they order, in addition to a standard delivery fee based on the delivery address.
- A 20% discount and VAT exemption shall be granted to all Senior Citizens & PWD patients with valid ID.
- All orders will be shipped within 48hours from the time order is confirmed and payment has been made by the patient.
- SII will only replace goods that were incorrectly or mistakenly delivered at no additional cost, provided products are in good condition and fit for resale. Returns will only be accepted within seven (7) days of receiving the package.
9. MISCELLANEOUS
- Modification. SII may, in its sole discretion, change these Terms with written or electronic notice. Unless otherwise provided, such amendments will take effect immediately and HCP’s continued use of this service provided by SII to HCP will be deemed HCP’s acceptance of the amendments. If you do not agree to any changes, you may cancel your enrollment to the Program.
- Force Majeure. SII shall not be deemed to be in breach hereof or liable to the HCP in any manner of any delay in delivery or other performance caused in whole or in part by, or otherwise materially related to, the occurrence of any contingency beyond SII’s control, including without limitation, fire, flood, pandemic, terrorist threats or acts, riot or other civil unrest, war, invasion, hostilities, strikes or other labor disputes, embargoes or transportation delays, shortage of labor, inability to secure fuel, energy, materials, supplies or power at reasonable prices from regular sources or on account of shortages thereof, delays or failures of any of SII’s suppliers to deliver, acts of God or of a public enemy, the effect of any existing or future laws, acts or regulation of any applicable government, or any other commercial impracticability.
